Hawaii’s concentrated insurance market makes provider enrollment and payer enrollment priorities clear for practices entering or expanding across the islands. HMSA and Kaiser Permanente account for roughly 90% of the commercial market, while QUEST Integration controls access to Hawaii’s Medicaid population.

1. HMSA: Your first commercial priority

Hawaii Medical Service Association (HMSA), the state’s Blue Cross licensee, operates the broadest commercial network in Hawaii. For most independent practices, HMSA belongs at the top of the enrollment sequence.

HMSA serves commercial, Medicare Advantage, Marketplace, and QUEST Integration members. A complete CAQH ProView profile supports the application process, but CAQH is not a substitute for HMSA’s own enrollment and participation requirements.

Before applying, confirm that:

That changes your approach. Do not treat Kaiser like a standard open-panel commercial payer. Contact Kaiser’s Hawaii provider relations or contracting team first and confirm whether your specialty, island, and practice location are accepting new providers.

  2. Specialty, subspecialty, and board-certification details.
  3. Hawaii service locations and appointment capacity.
