Virginia practices entering 2026 need a payer strategy built around provider enrollment and payer enrollment, not a one-plan-at-a-time scramble. Anthem, Sentara, UnitedHealthcare, and Virginia’s Cardinal Care plans form the core network priorities for many practices, but every product line has its own enrollment path, portal, and participation requirements.
1. Anthem: the statewide commercial priority
Anthem Blue Cross and Blue Shield, part of Elevance Health, remains a major commercial payer priority across Virginia. Its enrollment structure has three distinct tracks:
- Commercial plans: Enrollment is submitted through the Provider Enrollment application in Availity Essentials.
- Medicare Advantage: This requires a separate Medicare enrollment pathway, including active Medicare participation and applicable PECOS records.
- Medicaid: Anthem HealthKeepers Plus operates within Cardinal Care and requires Virginia Medicaid enrollment before plan participation.
A commercial Anthem approval does not place your practice into HealthKeepers Plus or Medicare Advantage. Treat every product line as a separate operational workstream. Confirm the provider type, specialty, tax identification number, NPI, locations, and effective date before billing.
2. Cardinal Care: DMAS enrollment comes first
Virginia Medicaid’s Cardinal Care program includes five managed care plans:
- Anthem HealthKeepers Plus
- Aetna Better Health of Virginia
- Humana Healthy Horizons
- Sentara Health Plans
- UnitedHealthcare Community Plan
The Virginia Department of Medical Assistance Services (DMAS) states that providers must enroll and periodically revalidate through the Provider Services Solution (PRSS) portal. Providers must also contract with managed care plans individually.
That creates a two-part enrollment sequence:
- Complete state enrollment through DMAS PRSS.
- Apply to the Cardinal Care MCOs that match your patient population and service area.
PRSS enrollment alone does not place you in every MCO network. Missing the MCO step can leave your practice appearing active with Virginia Medicaid while claims still deny for lack of plan participation.
3. Sentara: regional strength with statewide relevance
Sentara Health Plans is a major Virginia commercial payer, particularly important in Hampton Roads and eastern Virginia. It also participates in Cardinal Care through Sentara Health Plans Medicaid.
For practices serving coastal or southeastern markets, Sentara deserves early attention. Verify:
- Commercial versus Medicaid product participation
- Service-area availability
- Provider location requirements
- Specialty network status
- Effective dates and directory display

4. Other high-value commercial payers
Your 2026 payer list should also evaluate:
- UnitedHealthcare: Employer, Medicare Advantage, Medicaid, and marketplace products.
- Aetna: Important for employer-sponsored coverage and Medicaid, even as product availability varies by market.
- Cigna: Strong commercial relevance, with marketplace participation in selected Virginia areas.
- CareFirst: Especially important for Northern Virginia and the broader Washington, D.C., market.
- Kaiser Permanente: Concentrated in Northern and selected Central Virginia markets, with an integrated delivery model.

The highest-risk Virginia enrollment error is treating payer participation as one universal approval. It is not. Anthem commercial, Anthem Medicare Advantage, Anthem HealthKeepers Plus, and Cardinal Care MCOs require distinct tracking.
Build a payer matrix that records:
- Product line and state program
- Portal and submission date
- NPI, taxonomy, TIN, and locations
- Application status
- Effective date
- Revalidation deadline
- Directory confirmation
